SmartCare is an EU-funded project that aims to define a common set of standard functional specifications for an open ICT platform for the delivery of integrated care to older European citizens. It will be piloted in ten European regions, after 24 regions and their key stakeholders have defined a comprehensive set of integration building blocks around the challenges of data-sharing, coordination and communication.
